Educator Collection



Use this form when you want to check out a collection for use in your classroom.

Educator Collection is a service available to preschool, elementary, middle school, high school and home educators.  You may drop off or fax (330-364-8217) your completed form to your library.  You may also submit your completed form below.  Based upon your request, the library indicated above will pull a collection of materials specific to your needs and have it available for you to pick up.
